CHICOPEE Clinical Case Manager Job Description School Based Services
(SBS), part of the Positive Regard Network, invites applicants to apply
for the position of Clinical Case Manager (CCM), to provide intensive
counseling and support to students in a school setting through group
therapy, case management between school and home/ancillary services, and
support to the classroom team. These positions are generally scheduled
August through June during the school year, as a 10 month position,
working in a public school setting. We have three openings: Two
positions are scheduled 30 hours per week, Monday-Friday; there would be
flexibility to schedule the hours over four or five days during the
school day. The other position is a full time, 40 hours per week
position, temporary March through June. That position is Monday through
Friday, 7:30-3:30. While it is temporary, the position could turn into a
full time, regular position within the PRN in August. Position
Responsibilities: - Provides direct clinical service, e.g., individual,
group and/or family therapy as outlined in Individual Education Plans
(IEP\'s) and treatment plans of students. - Develops individual
treatment plans/behavioral plans for each student on caseload and
establishes clinical treatment goals and objectives to be included in
overall service plan for each student. - Provides emergency clinical
services including referrals based on psychiatric crises. - Provides
family outreach as needed. - Liaisons with outside agencies and
authorities, including social services organizations and school district
personnel, and represents school in a professional manner at community
meetings and events, including court, conferences, etc., as needed. -
May accompany students to school psychiatry appointments. - Writes
family assessments, psychosocial evaluations, clinical summaries, case
notes, IEP goals/benchmarks, progress reports, discharge summaries and
other clinical reports as requested or as needed. - Completes intake
procedures for assigned students including building history and
background files for incoming students and presentation of information
to staff during meetings. - Consults with and maintains ongoing
communication with Site Administrator, Educational Administrator,
Teachers, Floor Manager, and/or Lead Interventionists, as appropriate.
Informs staff of outside issues in a timely manner. - Assists in
development and implementation of transition plans for students changing
school settings. - Prepares clinical updates for presentation at
district team meetings and attends team meetings. - Administers clinical
assessments as needed. - Facilitates ongoing communication and attends
morning and afternoon staff meetings and/or treatment/behavior planning
meetings as needed and available. - Adheres to professional license
standards while upholding the school model of treatment, as
appropriate. - Maintains professional licensure and participates in
conferences, workshops or other courses for professional development, as
appropriate. - Mentors, trains and role models for new staff including
assisting with new staff orientation as requested. - Assists with
interventions as needed. - Assists students in the classroom setting
when appropriate and needed. **Please note that candidates who are

The position requires a Master\'s degree in Social Work, Psychology, or
related field; MA license in a clinical field (LMHC, LCSW, LICSW)
required, along with considerable experience in the field of counseling
(2+ years in a diverse clinical setting). At least 2 year(s) of relevant
experience preferred. Master degree preferred. Citizenship, residency or
