ACT Team Leader - JOB SUMMARY:

The Assertive Community Treatment Team Leader establishes, administers, and directs the program of Assertive Community Treatment (ACT), a self-contained clinical team that assumes responsibility for directly providing needed treatment, rehabilitation, and support services to identified individuals with severe and persistent mental illnesses; supervises and evaluates the multidisciplinary team in conjunction with appropriate psychiatric support to ensure service excellence and courteous, helpful, and respectful services to program individuals; and functions as a practicing clinician on the team.

ESSENTIAL FUNCTIONS:

  1. Direct the day-to-day clinical operations of the ACT team including to lead the daily organizational staff meetings and treatment planning meetings; continuously evaluate the status of individuals and do appropriate planning and coordination of treatment activities to ensure immediate attention to their changing needs.
  2. Direct and coordinate the admission process and treatment, rehabilitation, and support services of the program in coordination with the psychiatrist. Schedule the admission interview; develop and coordinate the initial assessment and initial treatment plan; assign the most appropriate staff to the individual treatment team (ITT); and provide clinical supervision of the development of the comprehensive assessment and the treatment plan for each individual.
  3. Direct and coordinate, for each individual, the person-centered comprehensive assessment of psychiatric history (e.g., onset, course and effect of illness, past treatment and responses, and risk behaviors), mental status, and diagnosis; physical health and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living (e.g., self-care, living situation, nutrition, money management); and family structure and relationships.
  4. Participate in staff recruitment, interviewing, hiring, work assignments, and orientation and performance supervision according to work rules, regulations, and policies and collective bargaining agreements; develop and implement staff orientation and training; and manage student training.
  5. Supervise medical records management assuring maintenance of the medical record in compliance with agency policies, Medicaid, and other third-party payment requirements; train staff on medical record requirements; regularly review client assessments, treatment plans, and progress notes written by the staff; and work closely with the Quality Assurance department.
  6. Develop and maintain program policies and procedures and revise, as necessary.
  7. Maintain a caseload of individuals in need of counseling services.

OTHER DUTIES:

  • Develop and participate in on-call and holiday service delivery.
  • Perform such other duties as assigned by supervisor.

QUALIFICATIONS:

  • Strong Clinical Skills and experience providing treatment to persons with severe and persistent mental illness.
  • Supervisory and program management experience preferred.
  • Clear understanding of the characteristics and problems of adults with severe and persistent mental illness and substance addictions.
  • Knowledge of Assertive Community Treatment Model.
  • Ability to develop and manage a clinical treatment team.

Minimum Requirements

EXPERIENCE/EDUCATION REQUIRED:

1.Masters Degree in nursing, social work, psychiatric rehabilitation, or psychology or is a psychiatrist.

2.Licensed or licensed eligible according to the laws of the Commonwealth of Virginia. Licensed staff preferred.

3.Valid Drivers License and copy of safe driving record.

4.Background check and fingerprinting required.
