We are looking for a compassionate and dedicated individual to join our team as a Homeless Case Manager. In this role, you will provide crucial support and assistance to individuals experiencing homelessness, helping them navigate complex systems and access the resources they need to stabilize their lives.Location: New York, NYHourly Rate: $30Responsibilities:Conduct comprehensive assessments to understand the needs and circumstances of clients experiencing homelessness.Develop individualized case plans in collaboration with clients, setting goals and identifying steps to achieve housing stability and self-sufficiency.Connect clients with appropriate community resources and services, including housing assistance, healthcare, mental health support, substance abuse treatment, and employment opportunities.Advocate on behalf of clients to ensure they receive timely and appropriate assistance from service providers and government agencies.Monitor progress towards goals, provide ongoing support and encouragement, and adjust case plans as needed.Maintain accurate and up-to-date documentation of client interactions, services provided, and outcomes achieved.Collaborate with other members of the support team, including social workers, counselors, healthcare professionals, and housing specialists, to coordinate services and ensure holistic care.Stay informed about relevant policies, programs, and resources related to homelessness and poverty alleviation.Qualifications:Bachelor's degree in social work, psychology, counseling, or a related field (Master's degree preferred).Previous experience working with individuals experiencing homelessness or in a similar social services role.Strong interpersonal skills and the ability to build rapport with clients from diverse backgrounds.Excellent communication skills, both verbal and written.Empathy, patience, and a nonjudgmental approach to working with clients facing difficult circumstances.Ability to work independently and as part of a team, managing multiple tasks and priorities effectively.Knowledge of community resources and social service programs available to individuals experiencing homelessness.Familiarity with case management principles and best practices.
