The Residential Home Coordinator serves as the liaison between the daily operations of our Residential Homes, which provide services for individuals with intellectual disabilities, and our Residential Program Manager.  This position is responsible for the overall daily functions, ensuring staff coverage, and documentation of the home they are assigned to, supporting and ensuring the physical, emotional, and psychological needs of the individuals in our care, maintaining the safety of our sites, and serving as the in-house expert for compliance, documentation and individual care. 

Some Essential Job Responsibilities include, but are not limited to:

  • Fulfills reporting/record keeping requirements of the job accurately, neatly and timely (including but not limited to: medication logs, Interdisciplinary Team notes, data sheets, health care plans, incident/observation reports, personnel logs, EIM reports, chore charts, petty cash records)
  • Monitors daily documentation done by staff ensuring accuracy, neatness, completeness, and thoroughness.
  • Responsible for overseeing the grocery purchasing, storage and food management processes including compliance with the monthly budget.
  • Conducts bi-weekly household inspections, and routine reviews to ensure cleanliness and safety of the residential site.
  • Maintains open communication with Residential Management pertaining, but not limited to, individuals in our care, health and safety concerns, compliance issues, staff performance, etc.
  • Collaborates with Director of Community Health to develop and monitor menus and ensures all nutritional requirements are met, calorie counts, food consistency, and other dietary restrictions and health maintenance are as ordered by the individuals PCP.
  • Serves as a member of the Interdisciplinary Team, assisting in the development of a comprehensive treatment plan which addresses the individual's needs and promotes their maximum independent functioning.
  • Assigned 20 -- 24 hours of floor time per week, adjusting accordingly in collaboration with Program Manager and operational needs.
  • Assists with meeting the needs of all individuals in a direct care capacity, such as personal finances, shopping, meal preparation, and other activities of daily living, which include bathing, toileting, dressing, housekeeping, laundry, etc.
  • Collaborates with direct care staff on providing the individuals in our care with fulfilling leisure, recreational, and social activities as well as community outings in accordance with the individuals likes and dislikes
  • Assists with training of new and current staff that work in the home to ensure that each employee has all skills and knowledge necessary to provide quality care to the individuals in the home.
