Case Manager (PT)

EAC Network, a not-for-profit social service agency that empowers, assists, and cares for over 62,888 people in need through 100 programs across Long Island and NYC, seeks a Case Manager to work part time for its In-home respite program.

The Case Manager position is a non-exempt position paying $20 per hour. Flexible schedule of 3-4 days a week between the hours of 9AM- 5PM, Monday-Friday for a total of 20 hours per week. The office is located in Garden City, NY and the role reports to the Program's Coordinator.

Primary Purpose of Job:

The Case Manager is responsible for providing case management support for caregivers who are caring for an older relative & assist with the day to day operations of the Program.

Principal Duties & Responsibilities:

  • Assist Program Coordinator and maintain a caseload of clients and monitor their participation in the program.

  • Maintain case documentation and client records.

  • Conduct initial home visits and re-assessments to monitor the provision of in-home Respite

  • Make referrals for entitlements and services to assist caregivers of older adults.

  • Assist with outreach to recruit companions and prospective clients.

  • Assist with screening and training of Respite Companions

  • Record telephone intakes and provide guidance and information for caregivers.

  • Process monthly fee collection records and review and submit time sheets.

    Knowledge, Skills, & Abilities Required:

  • Associates Degree required Bachelors Degree in human services or related field preferred.

  • Knowledge of resources and entitlements for senior citizens.

  • Must have excellent communication skills, both oral and written

  • Must be proficient in computer applications or other automated systems such as Microsoft Office Applications, email, and database software necessary to perform work assignments.

  • Must have valid New York State Drivers License and have access to an automobile

  • Prior experience working with the elderly preferred.
