SUMMARY:Provides clinical services to clients in the community at various locations including remote sites and client homes. Provides intervention and engagement, clinical assessment, and individual and group counseling to individuals with substance use disorders with the primary population being opiate use, but who also may have co-occurring mental health and medical disorders. Also provides coordination of care for client needs, education and harm reduction strategies for those client systems in a pre-contemplation/ contemplation stage of change, community outreach, and clinical services to family members of substance users. Works in collaboration with local treatment/service providers, SOR Project team members and the agency as a whole. Candidate must have an understanding of Hierarchy of Needs, Motivational Interviewing, Crisis intervention, Family engagement, and community outreach and case management skills. Travel is required and a varied schedule may be possible pending program development.Essential Duties and Responsibilities:1. Conducts comprehensive bio-psych-social assessments and develops intervention/ treatment plans.2. Provides direct counseling to clients and family systems.3. Provides education to the community and individual clients in a variety of settings.4. Coordinates client care through direct clinical engagement, counseling and case management.5. Work collaboratively with community agencies and inter-agency staff to assure effective referrals that address each client’s comprehensive needs.6. Collaborates with the Project Director to ensure service schedules and locations meet identified service needs; responsible for site set up for tele-medicine sessions.7. Oversight and coordination of peers at the direction of the Project Director to meet client needs and best practice is utilized.8. Documents all services provided in the electronic health record in accordance with NYS regulations including assessments, fee agreements, session notes, treatment plans, discharge summaries, etc to delineate the course of interventions and clinical process accurately and timely.9. Maintains trip roster, engagement log and ensures peer advocate documents are complete before the end of the day.10. Train community members, clients and families in Narcan.11. Refer clients to the appropriate level of care and work collaboratively with the Care Manager and respective treatment providers.12. Remains up to date on community services that are offered for Substance Abuse Treatment, insurance navigation, medical providers, mental health providers, homeless services, transportation, etc.13. Makes recommendations for changes in the service delivery for the grant based on community need.14. Attend meetings, workshops and trainings as requested.15. Performs all other tasks as assigned.Working conditions and physical demands:Potential for unpredictable clients or clients under stressful situations that require consistent support. Must have the ability to climb stairs, moving freely (bend, stoop, stand, walk). Travel is required; must have valid driver’s license and reliable vehicle. Mobile units are in the community and may present environmental risks at times including bad weather and unpredictable clients.Qualifications:Education and/or experience:Master’s Level QHP (preferred) or CASAC with 5+ years experience in Substance Abuse treatment, prevention or related field. Home and community based service provision desired.Skills, Licenses, and/or competencies:Excellent oral, written communication, proficient organizational skills, ability to work as a team and collaborate professionally with other providers, proficient computer skills (experience with an EHR a plus). Training and/or practice experience in Motivational interviewing and client centered care.
