Purpose : Establish and maintain the development, implementation, management, compliance, and evaluation of funding designated for AIDS Institute funded case management grants (ARCH/PAWH and Ryan White B) . In conjunction wit h the Director, Case Management, develop and supervise applicable aspects of the grant and provide guidance and leadership to case management team members .

  • Service Development and Evaluation

  • Co-lead in the administrative, planning, development, and implementation functions of each grant.

  • Develop and maintain relationships with community agencies and medical facilities to obtain new referrals and ensure unduplicated services for existing patients/clients .

  • Work with department leadership to design and monitor performance and quality standards, including outcome evaluations, productivity and workload targets, peer review and client/patient record reviews .

  • Ensure that staff members meet the contract requirements, deadlines and expectations of the agency, our department and our funders as program .

  • Work with department leadership to address patient complaints, identify possible/probable root cause, and take steps necessary to prevent recurrences in the future .

  • Assist in developing , updating, and maintain ing all applicable policies and procedures in accordance with Grant funding requirements.

  • Supervision and Performance Management

  • Participate in the selection , hiring and orientation of case management staff working in the grants.

  • Ensure staff development, assess competencies, and provide guidance as needed.

  • Clearly articulate expectations and manage to those expectations through positive feedback, coaching and progressive discipline if required .

  • Management of staff time clocks, work schedules, etc.

  • Review of documentation that is completed on behalf of and about patients/clients. Includes assessments, service plans and log notes.

  • Address performance issues through coaching/ counseling, performance improvement plans, and progressive discipline as required .

  • Demonstrate leadership, good judgment and sound decision making when handling situations that require management intervention.

  • Complete yearly performance evaluations for assigned direct reports.

  • Adhere to all government and funder regulations.

  • Assist in identifying additional resources that help support the needs of pa tients/clients and staff .

  • Assist in the completi on of necessary program reports/audits as applicable .

  • Responsible for the develop ment of policies and procedures to meet grant and agency requirements .

  • Monitor grant deliverables and budget, implemen t strategies to successfully execute as applicable .

  • Support all communications to the g rant contractors via reporting, calls, and meetings on progress as applicable .

  • Develop p artnerships/ MOU’s with community organizations to support training and service delivery .

  • Reporting through management chain on progress and status of the team.

    Qualifications

  • Bachelor's degree in social work , h uman s ervices or closely related field.

  • At least 2 years of experience working in a management/supervisory role within a s ocial service, nonprofit, grant management, or healthcare setting preferred .

  • Familiarity with regional HIV primary care, mental health, substance abuse and other services and resources.

  • Possess an understanding of community level work and the importance of collaborating and coordinating with other organizations.

  • Effective communication and documentation skills.

  • Cultural and linguistic competence for the target population.

  • Must have a positive attitude, demonstrate initiative and professionalism.  

  • Must be accustomed to diversity.

  • The ability to maintain confidentiality is essential.

  • Experience in managing client case load assignments and functioning within a management team.

  • Must be able to work flexible hours including weekdays, evenings and weekends when needed. Must have a reliable vehicle, a valid NYS driver’s license, and proof of required automobile insurance.

    Physical Requirements

    While performing this job, the employee must stand, sit, and walk.   Occasionally the employee must stoop, bend and lift or move up to 25 lbs.
