MEDICAL ASSISTANT-PHYSICIAN REFERRALS and INS. VERIFICATION

Job Summary and Responsibilities

[Facilitates referral program, patient follow-ups and health screenings from our Urgent Care department.

[Performs tasks like: Coordinate patient care and post-visit planning processes with other hospital departments, external service organizations, agencies and healthcare facilities to achieve the defined patient outcomes;Implements and facilitates all patients' discharge plans including referrals, post visit testing and/or procedures;Collaborates with patients, families, and significant others to provide education and identify post-visit needs; Assists in achieving maximum benefits from Pinnacle healthcare services;

Requirements

MINIMUM REQUIREMENTS:

• High school diploma or equivalent
• Successful completion of Medical Assistant program
• One (1) or more year as Medical Assistant
