Our client is seeking a dedicated Community Support Navigator to join their team. In this role, you will be responsible for providing a continuum of consumer and community-based information & referral (I&R) and independent living (IL) services across Disability Network Washtenaw Monroe Livingston (DNWML) service areas.

This role is responsible for:

  • Providing IL oriented services including time management, healthy relationships, boundary setting, building supportive social networks, cooking, travel, financial management, etc.
  • Assisting primary constituents of DNWML to achieve independent living and economic self-sufficiency goals using Peer Support, Motivational Interviewing, and Person-Centered Planning approaches.
  • Coordinating access to community resources required to achieve consumer IL goals and maintain social support.
  • Designing, developing, implementing, and managing social and leadership groups, programs, and other opportunities for and with consumers.
  • Collaborating with others to develop community partnerships for leadership, system advocacy, and civic participation opportunities for consumers.
  • Providing Individual and Systems Advocacy, IL services, and/or assistance to parents, teachers, students, and/or designated schools as needed.

This position reports to: Director - Monroe

The ideal candidate will have:

  • High School Diploma or GED (required), Associate's Degree (preferred)
  • 1-2 years of experience in related work in human services and/or educational history
  • Personal experience with disability and/or knowledge of disability issues and the consumer/community-based IL philosophy

Responsibilities:

  1. Delivery of Independent Living Services
  2. Delivery of I&R Services for Youth and Adult Consumers

Requirements:

  • Degree: High School Diploma or GED (required), Associate's Degree (preferred)
  • Field Experience: 1-2 years of related work in human services and/or educational history
  • Software Experience: Knowledge of standard office software
