JOB DESCRIPTION

Position:\ Social Services Designee

Department:\ Social Services

Review Date:\ March/2018

Purpose:

The Social Services Designee plans, organizes, and implements the overall operation of the Social Services Department in accordance with Federal, State, and Local Standards, guidelines, regulations and facility policies and procedures, to assure that the medically related psychosocial needs of the residents are met and maintained on an individual basis.

Report to: Administrator

Primary Job Duties: (*denotes Essential Job Functions)

*1.\ Plans, organizes, implements, and evaluates the social service program for the facility under supervision of contracted social worker.

*2.\ Develops written plan of care (Preliminary and comprehensive) for each resident that identifies the problems/needs of the resident and the goals to be accomplished for each problem/need identified.

*3.\ Implements and maintains social service care plans, resident social assessments, initial social history, annual updates, and other required planning and documentation activities in accordance with Federal, State and Local guidelines, regulations and facility policies.

*4.\ Interviews residents/families and necessary and in a private setting to obtain information concerning the residents personal and family history, health history, etc. and develops a social service record.

*5.\ Participates in determination of appropriate placement of resident prior to admission into facility. Work closely with other facilitys discharge planners to coordinate new admissions. Meets with all residents and families at admission to obtain all necessary copies of documents and insurance cards, and complete all admission paperwork. Establish new record in HER and scan all documents.

*6.\ Explains and assists in the completion of appropriate forms and answers resident and family questions.

*7.\ Develops and maintains professional relationship with the medical profession, other health-related facilities and organizations, appropriate social service agencies, Refers residents/families to consultants, therapists, social service agencies and other resources when facility does not provide needed services.

*8.\ Participates in discharge planning, contacts appropriate agencies, and counsels residents and families.

*9.\ Provides information to resident/families as to Medicare/Medicaid, and other financial assistance programs available to resident.

*10.\ Provides consultation to facility staff members, community agencies, etc., in efforts to solve the needs of the resident through the development of social service programs.

*101.\ Schedules regular visits to area health-related facilities and provides current information about facility programs and services and availability of beds.

*12.\ Works with facility consultants as necessary and implements recommended changes as required.

*13.\ Assists in the development of department budget and monitors budget on a monthly basis to ensure social services costs are maintained within budgetary boundaries.

*14.\ Implements and maintains an effective orientation program for new residents and family members.

*15.\ Creates and maintains an atmosphere of warmth, personal interest, and calmness throughout the department and facility for residents, staff, families, and visitors.

*16.\ Meets with staff on a regular basis to identify and correct problem areas and to improve services.

*17.\ Initiates and maintains contact with residents families, involving them with non-medical progress reports as necessary.

*18.\ Serves on various facility committees as required.

*18.\ Develops and participates in regularly scheduled orient tion and in-service training programs relating to the social, emotional, and medical needs of the residents.

*20.\ Performs all duties in a safe manner and ensures department staff members performs their jobs safely, following all infection control procedures.

*21.\ Maintains confidentiality of resident care information.

*22.\ Informs the resident/family of residents personal, property and other rights.

*23.\ Assists and counsels families with funeral planning and other matters relating to resident deaths.

*24.\ Implements and maintains an ongoing quality assurance program for the Social Services Department.

*25.\ Assists in the development of a plan of correction for social services deficiencies noted during survey inspections and provide a written copy to the Administrator.

*26.\ Assists in the scheduling and provision of transportation for residents to special events, doctors visits, etc.

*27.\ Manages new inquiries, visits potential residents in their homes and assists with other marketing responsibilities.
