following opportunity. Description Lutheran Social Services of WI &
Upper MI is currently seeking a Service Coordinator to serve our tenants
in the Sparta Arms Apartments in Sparta and the Tom Woodman Villa in
Richland Center. The Service Coordinator program is designed to promote
and support self-sufficiency and independence of those individuals
residing in properties, facilitating a variety of support services
within the residential community. Our Service Coordinators connect
residents with resources in the community, provide monthly presentations
on issues in the community, and connect residents with recreational
programming. This is a 36 hour and benefit eligible role , the position
will be located onsite on the property! The position would be based in
Sparta and require travel every other week to Richland Center. Under the
direction of the Program Supervisor, the Service Coordinator HUD
provides information and referrals to community connections to tenants
following guidelines, as defined in the HUD Housing Handbook Section
202, who need supportive community services to maintain
self-sufficiency. Essential Duties and Responsibilities: This list of
duties and responsibilities is not all-inclusive and may be expanded to
include other duties and responsibilities, as management may deem
necessary from time to time. Provide general informal case management,
including intake, and referral services to all tenants needing such
assistance when from case management is not available. Assessment of
health, psychological and social needs. Development of an individually
tailored case plan for services and periodic reassessment of the
tenant\'s situation. Establishes linkages with all agencies and service
providers in the community: shops around to determine/develop the best
