Accord is looking for a driven leader to join our team as a Sr. Director of Care Management. In this role, you lead, plan, develop, implement, and grow care management and care coordination programs that reflect the needs of communities, adhere to best practice and evidence-based models, and are fiscally viable.

Additional responsibilities include:

  • Work with department leaders to develop and implement processes that align and comply with all contracting or licensing requirements, accreditation standards, and Accord’s mission, vision, and values.

  • Manage and supervise a high performing team poised for growth.

  • Meet regularly with supervisees to ensure timely feedback, answer questions, and identify trends or issues such as training and other resource needs.

  • Measure, manage, report and drive employee productivity using EHR, Excel, and other data tools; ensure accountability, promote performance, and meet business objectives.

  • Assure the fiscal viability of programs including seeking new funding sources, building and maintaining positive relationships with funders, and utilizing effective marketing strategies.

  • Gather and analyze qualitative and quantitative data and measurements for decision-making and evaluation of program effectiveness.

  • Using person-centered practices, coordinate services for individuals with disabilities among different health and social service professionals and across settings of care.

  • Integrate and improve care by:

  • Ensuring optimal health status and decreasing the rate of health decline.

  • Providing social or community support systems.

  • Promoting a safe environment.

  • Reducing or, if possible, eliminating the impact of behavioral health issues.

  • Encouraging self-reliance.

  • Provide direct services, including care management and care coordination services to assess, plan, facilitate and advocate for options and services to meet an individual’s health needs, through communication and available resources to promote high-quality, cost-effective health outcomes.

  • Advocate for individuals, including awareness of and sensitivity to culturally appropriate care.

  • Apply ethical practice principles such as respect for the autonomy, dignity, privacy and rights of the individual.

  • Promote collaborative, proactive, and person-focused relationships.

  • Develop and oversee the program budget, including identifying potential funding sources and managing expenses.

  • Work with the Electronic Health Records (EHR) Administrator to set up and maintain program records.

  • Serve as a representative of the agency to the people we support, our community partners and external constituents. Serve as a positive role model for other employees.

  • Present to committees, boards, commissions, funding sources and other community groups and represent the agency on outside advisory boards, committees and commissions.

Requirements

Education:

  • Bachelor’s degree AND be licensed as a Qualified Professional in the state of MN in one of the professions as defined below:

  • Licensed social worker (LSW)

  • Registered nurse (RN)

  • Nurse practitioner (NP)

  • Physician

  • Mental health professional (MHP)

  • Masters Degree in related fields preferred.

Skills & Experience:

  • Position requires a minimum of 8 years of work experience in the field of human services working with older adults and/or people living with disabilities.

  • Minimum of 8 years of experience in a supervisory capacity.

  • Knowledge of health, social services, and funding sources that support individuals.

  • Experience in program development and implementation, preferably with a focus on services for individuals with disabilities.

  • Experience in budget development and management.

  • Self-starter with ability to work independently and as part of a team.

  • Strong organizational skills.

  • Proficient in GSuite, Excel, and third party analytics.
