Assess & monitor the needs of the consumer & implement a service plan which is following the Executive Office of Elder Affairs & Elder Services of Cape Cod & the Islands Home Care Program regulations & contract obligations. Duties & Responsibilities: Conduct an initial assessment of the consumer\'s physical, social, environmental, & emotional status to determine needs, income, & age eligibility. Develop a service plan & supportive networks through contact with the consumer, those involved with the consumer (family members, physician, clergy, etc.) & in consultation with the Supervisor. Assist the consumer to obtain & utilize other community services such as a certified agency, housing, fuel assistance, council on aging, MassHealth, etc. Provide education & support to the consumer & those directly involved with consumer?s support network. Provide intervention services when appropriate. Maintain consumer records, ensuring compliance with Executive Office of Elder Affairs & Elder Services of Cape Cod & the Islands. Attend assigned Team meetings weekly & other meetings as needed. Contacts are made on a regular basis with the entire range of Agency personnel. Contacts made on a regular basis with members of the community, service providers, consumers/caregivers, & their families. Other duties as assigned. Skills & Abilities: Ability to collaborate, take initiative, provide/ receive feedback. Ability to communicate & work with the elderly & community resources serving the elderly population. Excellent computer skills required. Ability to work independently & professionally. Valid MA driver\'s license & car required Education, Licensure & Experience: Bachelor\'s degree in social work, human services, nursing, psychology, sociology or related field from an accredited college or university. OR Bachelor\'s degree in another discipline with experience or strong interest in in human services via previous employment or volunteer activities Physical Requirements: Prolonged periods sitting at a desk & working on a computer. Must be able to lift up to 15 pounds. Travel Required daily throughout the service area. Travel to training & meetings as needed.
