Job Opening: Pediatric Mental Health Care Coordinator, TCHATT, Department of Psychiatry

Work in an inter-professional model and collaborate with pediatric psychiatrists, psychologists, other licensed professional counselors, and administrative staff to identify the long-term psychological needs of patients. Assemble information concerning patients clinical background and referral needs.Connect students and families to community resources for long term care, providing ongoing support and facilitation to ensure effective transition. Coordinate with TCHATT professionals, schools, and families to minimize obstacles and increase patient and provider satisfaction. Maintain confidentiality of information as appropriate in all aspects of care.Oversee communications related to referral information among team members, patients, and school staff. Update electronic medical records. Support team members in disseminating appropriate referral information.Maintain community resources database for successful external referrals.Other duties as assigned.
