License: Asbestos Training Provider

An Asbestos Training Provider is any person or other legal entity, however organized, who conducts some or all of the training programs for asbestos professional disciplines which are regulated in Arkansas Pollution Control and Ecology Commission Regulation 21, Asbestos Abatement. Requirements: 1) Must submit a completed application on a form provided by the Department. 2) Training provider fee required. 3) Must submit a statement certifying that each course complies with the requirements of Title 40 of the Code of Federal Regulations (CFR) Part 763, Appendix C to Subpart E Model Accreditation Plan (MAP) and Arkansas Pollution Control and Ecology Commission Regulation 21, Asbestos Abatement. 4) Must provide resumes of all instructors. 5) Must provide sample course agendas. 6) Must provide a completed Disclosure form for firms. 7) Must provide copies of manuals for each class (at ADEQ staff discretion). License fee: $375.00
