IC 25-27-1-6.1Requirements for physical therapist license Sec. 6.1. An applicant for a license as a physical therapist must do the following:(1) Complete the application process, including payment of fees.(2) Submit proof of graduation from a professional physical therapist education program accredited by a national accreditation agency approved by the board.(3) Pass an examination approved by the board.(4) Submit to a national criminal history background check under IC 25-1-1.1-4.(5) Meet any other requirements established by board rule.
