Applicants must: 1) Be at least 18 years old. 2) Completed a curriculum for physical therapist assistant approved by the State Board. 3) Pass a national examination. 4) Submit fingerprints, photo, original transcripts (including high school) and references with application. Notes: Application signature must be notarized. The physical therapist assistant curriculum must be at least a 2-year program requiring a minimum of 60 semester hours at an accredited school. A minimum of 1.5 continuing education units are required for physical therapist assistant license renewal. Fees may be subject to change. Fees are: application and initial license--$200; national examination--$370; testing center fee--$70; examination processing fee--$25; fingerprinting--$37.50; annual license renewal--$100.
